Recanalization of infrapopliteal occlusions eview

چکیده
Crural vessel occlusive disease is a common finding in patients presenting with symptomatic chronic critical limb ischemia. This patient group has poor treatment prospects with high morbidity and mortality. Furthermore, infrapopliteal vascular disease is by its very nature technically challenging to deal with by endovascular or surgical techniques. The multilevel distribution of the disease process may be appreciated and assessed by multidetector computed tomography or magnetic resonance angiography prior to intervention, to determine the approach to treatment. Many patients are candidates for nothing more than conservative treatment. In the remaining patients, transluminal and subintimal angioplasty achieve good clinical success and limb salvage with low complications. When used in this anatomical territory these techniques require: meticulous attention to detail; a sound knowledge of suitable balloon, catheter and wire combinations; and familiarity with techniques to deal with complications.

Endovascular Treatment of Multilevel Chronic Total Occlusion Using a Stent Puncture Technique in Buerger's Disease
We reported a patient with Buerger's disease who presented with critical limb ischemiawith prior recurrent occlusions after multiple surgical and endovascular treatments. Total occlusion of the whole native femoropopliteal and infrapopliteal arteries was observed. The femoropopliteal bypass graft, as well as a stent that was implanted in the mid-popliteal artery, were also occluded. Because of ...
متن کاملRetrograde approach for endovascular salvage of an infrapopliteal vein bypass.
Endovascular treatment through femoropopliteal and infragenicular percutaneous transluminal angioplasty, both in native vessels and in bypass salvage, has been an emerging technique in recent years. However, in some cases, a difficult anterograde access in distal occlusions has limited the technical success of this procedure. Combined subintimal arterial flossing with antegrade-retrograde inter...
متن کاملOcclusion Location of Middle Cerebral Artery Stroke and Outcome after Endovascular Treatment.
BACKGROUND The aim of this study was to analyze the influence of the location of middle cerebral artery (MCA) occlusion on recanalization, complications and outcome after endovascular therapy. METHODS Four-hundred sixty-four patients with acute MCA occlusions were treated with endovascular therapy. RESULTS Two-hundred ninety-three patients had M1 occlusions, 116 had M2, and 55 had M3/4 occl...
متن کاملInfrapopliteal application of paclitaxel-eluting stents for critical limb ischemia: midterm angiographic and clinical results.
PURPOSE To report the midterm (<or=1 year) angiographic and clinical outcomes of a prospective study investigating the infrapopliteal application of paclitaxel-eluting stents (PES) in patients with critical limb ischemia (CLI). MATERIALS AND METHODS Infrapopliteal angioplasty was chosen as first-line therapy in patients with unilateral or bilateral CLI and additional femoropopliteal angioplas...
متن کاملAn Effective Guidewire Looping Technique for the Recanalization of Occlusive Segments of Infrapopliteal Vessels
OBJECTIVE To determine the efficacy, safety and primary follow-up results of a guidewire looping technique for the treatment of infrapopliteal arteries. MATERIALS AND METHODS From October 2006 to May 2008, an intraluminal angioplasty of the infrapopliteal arteries was attempted in 200 consecutive patients. Altogether, 417 infrapopliteal lesions, with lengths varying from 2 cm to 32 cm, were t...
